What to Bring to Appointment

Attention Uber and Lyft drivers! In order to access your mileage correctly and take advantage of the different rate starting from 07/01/2022, it is important to keep accurate records of your mileage for the entire year, split into two parts: 01/01/2022-06/30/2022 and 07/01/2022-12/31/2022. Have access to your mileage log online, as well as paper logs and other mileage tracking devices, and make sure to report them split into these two parts for easy record keeping.

Basic General Information

  • Social security or ITIN numbers plus birth dates for everyone on your tax return
  • Driver’s licenses or state-issued IDs
  • Estimated tax payments for the year
  • Voided check or account and routing numbers (only for direct deposit of tax refunds)

Income and Source Document

  • W-2 forms from all employers
  • 1099 forms for any freelance or contract work
  • Records of any self-employment income
  • Records of any rental property income
  • Records of any investment income
  • Records of any unemployment income
  • Records of any retirement income
  • Records of any Social Security income
  • Records of any alimony received
  • Records of any other sources of income
  • Records of any capital gains from the sale of personal property

Proof of Resindence for Claiming Various Child Credits

  • School Record or Statement
  • Landlord or Rental Agreement
  • Statment from Health and Welfare indicating kids names
  • Social Services Statment or record
  • Tuition, Form 1098, Education Realated Expenses
  • Childcare expenses and the provider’s tax ID number
  • Offical letter from emploeyment or reputable agency stating kids names and address

Healthcare

  • Health Insurance Marketplace Statement (Form 1095-A) if applicable (Dont forget this one!)
  • Health coverage statements or marketplace exemption certificate if applicable
